Informed Consent, Disclaimer, Release of Liability, Client Responsibility
Client Agreement
This agreement (the “Agreement”) is made between the Client and Julie King of Breath Works Miracles and Trans Breath (“Instructor”) effective as of the enrollment date.
Nature of Program:
The program involves the analysis of the Client’s life experiences with information that is spiritual in nature. The purpose is to provide a broader perspective on the connection between the mind (emotions and thought patterns), body, and soul. The feedback provided by the Instructor is for informational purposes only and is not intended to substitute for medical or psychological diagnosis and treatment.
Responsibility and Liability:
The Client bears the sole responsibility for any decision or action taken based on the information provided by Breath Works Miracles and Trans Breath. The Instructor, Julie King, cannot be held liable for any loss or injury resulting from the direct or indirect use of the information. The Instructor makes no absolute claims, guarantees, representations, or warranties regarding the completeness, accuracy, or reliability of the guided information and recommendations.
Implementation of Recommendations:
The Client acknowledges that it is their choice to follow or not to follow the recommendations provided. It is the Client’s sole responsibility to implement the recommendations as outlined and in a timely fashion to support their personal development.
Disengagement from Instructor:
The Instructor reserves the right to disengage from the Client as an Instructor if the Client fails to fully disclose the use of other therapies, modalities, herbs, supplements, illicit drugs, excessive alcohol consumption, or pharmaceuticals that could interfere with the training program and adversely affect the Client’s health. The Instructor may also disengage from the Client if they become argumentative, combative, or willingly alter or disregard the recommendations in a manner that disrupts the Three-Month Personal Development Training Program.
Medical Advice:
The Instructor recommends that the Client continue seeing their medical doctor or licensed healthcare professional for any physical or psychological ailment. The Client agrees to provide honest and complete information about their medical conditions to ensure safe participation in the program.
Medical Contraindications:
The Client affirms that they have disclosed all known medical conditions and understands that Breath Work may be contraindicated for certain medical conditions.
Updated Medical Profile:
The Client agrees to keep the Instructor updated on any changes in their medical profile, and there shall be no liability on the Instructor’s part if the Client fails to do so.
Physical Comfort and Safety:
If the Client experiences any pain or discomfort during the training, they agree to inform the Instructor immediately.
Non-Medical Diagnosis:
The Client acknowledges that the Instructor, Julie King, is not qualified to diagnose, prescribe, or treat any physical or mental illness, and nothing said during the training should be construed as such.
Professional Conduct:
The Client understands that any illicit or sexually suggestive remarks or advances will result in immediate termination of the training without refunds.
Session Etiquette:
The Client agrees to contact the Instructor at scheduled times and to provide 24-hour notice for session cancellations or rescheduling.
Client Guidelines for Breathwork Sessions
To ensure a safe and effective Breathwork experience, please adhere to the following guidelines:
- Alcohol and Intoxicating Substances: Refrain from consuming any alcoholic beverages or intoxicating substances at least 24 hours prior to the Breathwork session. These substances can interfere with the breathing process and may compromise the effectiveness of the session.
- Comfortable Position: During the Breathwork session, clients are invited to choose a comfortable position. You can either lie down or sit on the floor or bed, whichever feels more relaxing for you.
- Potential Physical and Emotional Sensations: Be aware that Breathwork can evoke various physical and emotional sensations. It is not uncommon to experience tingling, dizziness, trembling, shaking, and emotional releases through tears. These sensations are normal and part of the transformative process.
- Control Over the Breath: Clients have complete control over their Breathwork sessions. If at any point during the session you feel overwhelmed or uncomfortable, you have the freedom to return to your regular breathing pattern.
- Self-Care and Mindfulness: Each client is responsible for their own self-care during the Breathwork session. Pay attention to your body and emotions, and practice self-compassion throughout the process.
Remember, the goal of Breathwork is to facilitate relaxation, conscious elevation, and personal development. By following these guidelines, you can maximize the benefits of the session and create a positive and transformative experience.
I, the Client, hereby acknowledge and understand that Meditation, Conscious Breathwork, Life Coaching, and Reiki are complementary healing modalities that do not serve as substitutes for medical interventions, body therapy, or psychotherapy. I am aware that Julie King, as a master teacher, may offer suggestions for self-care and make appropriate referrals as needed.
I recognize the importance of open communication between myself and Julie King to enhance mutual understanding and acceptance of the energy work provided during the treatment sessions.
I, the Client, am fully aware that Meditation, Conscious Breathwork, Life Coaching, and Reiki can offer numerous benefits, including diminished pain sensations, increased relaxation, relief from anxiety, and an enhanced sense of well-being. However, I understand that the effects may vary depending on my individual response patterns.
While there are no known harmful effects from these modalities, I agree to hold Julie King harmless from any possible effects that may cause temporary physical or emotional discomfort. I take full responsibility for my self-care and personal development throughout this process.
I, the Client, understand that I am in complete control of my own body, and I have the autonomy to “stop” any session or treatment at any time if I feel the need to do so.
Payment and Cancellation:
The Client agrees to pay the entire program fee either as a single payment prior to the initial strategy session, or in three monthly installments. The fee is non-refundable, and cancellation can be made within 24 hours of program enrollment.
